sustain the member natural support network.

Community Living Supports
Daily support to help members live as
independently as possible in the community.

Community Transition
Support moving from institutional or higher-level
care back into community living.

Individual & Transitional Support
Targeted support for major life transitions
including aging out of foster care and housing
changes.

Respite
Temporary relief for primary caregivers to
sustain the member’s natural support network.

Supported Employment
Job development, coaching, and long-term
workplace support under the 1915(i) framework.

Independent Living Program

Services

For foster youth ages 18-21

Housing Support
Member-selected housing with dignity by design
— housing is separate from services.

Weekly Therapy
Built into the program — in person or virtual —
no separate referral needed.

Financial Literacy
DSS stipend goes directly to the member. They
pay rent, save a portion, and HS&M matches
savings at graduation.

Supported Employment
Job search, interview prep, and workplace
navigation.

Education Track
GED, trade program, or college enrollment
required — supporting EBT eligibility and career
development.

  1. Submit a Referral or Inquiry
    Care managers, hospitals, shelters, discharge
    planners, HCBS providers, individuals, and
    families are all welcome. Contact us at

  2. Eligibility Confirmation
    HS&M confirms eligibility and service authorization before services begin
  3. Onboarding
    Each member receives a Direct Support Person
    and a Qualified Professional. Clear short-term
    goals are established together from day one.
